I agree to indemnify and defend Radha Krishna Temple of North Carolina (RKTNC), its teachers, assistants, third party providers, volunteers, officers, and distributors and employees against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my participation in the activities and use of the property of Radha Krishna Temple of North Carolina (RKTNC). I agree to pay for all damages to the property or facilities of Radha Krishna Temple of North Carolina (RKTNC) caused by my negligent, reckless or willful actions.

I, my heirs, or legal representatives, do hereby waive and release and agree not to Radha Krishna Temple of North Carolina (RKTNC), its teachers, assistants, third party providers, volunteers, officers, and distributors and employees of any and all liability and responsibility from injury, accident, illness, death, legal and medical fees sustained now or in the future resulting from my participation in any activity.

I certify that I am physically well and suffering from no medical problems, conditions, impairments, diseases, or any other illness that would prevent my participation or increase my risk of injury and/or illness as a result of partaking in the Program and do hereby assume all responsibility for my participation in any exercise, yoga, meditation or activity associated with Radha Krishna Temple of North Carolina (RKTNC).

I agree that Radha Krishna Temple of North Carolina (RKTNC) is in no way responsible for the loss or damage of any of my belongings while I attend the program.

I agree to allow Radha Krishna Temple of North Carolina (RKTNC) to use my name and likeness in a photo or on videotape, if any, for promotional purposes or media releases.

I give my consent to be added to the Radha Krishna Temple of North Carolina (RKTNC) email list. I also hereby override NDNC registration and permit Radha Krishna Temple of North Carolina (RKTNC) and its authorized representatives to contact me using my phone number and Email ID.
